Was it on London?
Because I think that some form of connection is derped between that server and SourceBans.
I guess that derp folds over into the SteamRep plugin too.

I think also the Dallas and NY servers have bad connection to SourceBans.

Dallas doesn't even have the SteamRep Checker plugin installed.
